Chronic Care Management
Our goal with chronic care management includes long-term treatment for Hyperlipidemia, Diabetes, and Heart and Thyroid diseases. These diseases can be expensive for the individual and the surrounding community if not treated because of the end result conditions such as stroke, kidney failure, and/or a heart attack. We aim to stabilize these conditions that our patients may face by providing consistent care
to prevent an emergency.
